UL陰性未分化型胃粘膜内癌の術前診断―内視鏡診断―通常観察を中心に

Diagnosis of Undifferentiated-type Early Gastric Carcinoma by Conventional Endoscopy

要旨　内視鏡的粘膜下層はく離術（endoscopic submucosal dissection ; ESD）の適応拡大が検討されている未分化型胃粘膜内癌の内視鏡診断について，通常観察を中心に述べた．未分化型癌の存在診断にあたっては，胃固有腺領域の褪色陥凹性病変に特に注意して観察することが重要である．粘膜内にとどまる未分化型癌では，陥凹底は平滑で凹凸不整はなく周囲隆起も認めない．また分化型癌と比べ，粘膜の中間層から深層を這うようにして浸潤する頻度が高いこと，非連続性に進展する場合があることにより，病変境界が時に不明瞭となることがある．内視鏡診断の際にはインジゴカルミン撒布によるコントラスト法の併用や観察する方向や距離，観察時の空気量を変えるなど注意深い診断が重要である．

　An expanded indication for endoscopic treatment of undifferentiated-type early gastric adenocarcinomas has been suggested. To detect undifferentiated-type early gastric adenocarcimomas, it is important to watch discolored depressions in the area of gastric peculiar glands. Intramucosal undifferentiated-type gastric carcinoma doesen't have marginal elevation. The base of depression is smooth. Because undifferentiated-type adenocarcinomas don't extend continuously and extend in the middle or deeper layer of mucosa, the borderline of undifferentiated-type gastric adenocarcinomas is often unclear. So, it is important to observe lesions by changing angles, distances and amount of air or using the dye spraying method.
