Follicular lymphoma (FL) predominantly involves lymph nodes. Primary FL of nonhaematopoietic extranodal sites except the gastrointestinal (GI) tract and the skin is not common. FL is a neoplasm of follicle centre B cells〔centrocytes/cleaved follicle centre cells (FCC) and centroblasts/noncleaved FCC〕, which has at least a partially follicular pattern. Virtually all cases of FL have cytogenetic abnormalities. The most common abnormality is t(14;18)(q32;q21) that is associated with over expression of BCL2 and lymphomagenesis. Most cases with t(14;18) have additional breaks which have been shown to be associated with transformation in FL. One study substantiates the recent gene expression profiling data highlighting the importance of nonneoplastic cells in determining prognosis in FL.
