要旨　4型胃癌治療の約20年を回顧した．手術単独の治療成績は進歩がみられたが，現在ある一定のレベルに達しているものと考えられる．術前術後化学療法との相乗効果で今後さらなる予後の改善を期待することができる．しかし高度に進行した4型胃癌は依然治療抵抗性の場合も多い．将来的なperspectiveを備えた臨床試験を実施するとともに，分子生物学的な知見を臨床に取り入れていく必要があるだろう．

　The trend of surgical treatment for type 4 advanced gastric cancer was studied retrospectively. A total of 696 patients who underwent surgical and medical treatment between 1985 and 2003 were included in this study. Surgical outcome has progressed over these twenty years, but now it has already reached a plateau. Overall survival of the patients with type 4 advanced gastric cancer will be better with combined therapy of surgical treatment and pre and post operative chemotherapy. Well designed clinical trials and collaborative work with molecular biology are needed.
