 The status of the resection margin and the risk of nodal involvement should be integrated into the treatment policy following endoscopic removal of cancer invading into the submucosal layer. A definite requisite resection margin is still under discussion, and there have been various opinions regarding the definition of resection margin involvement, including 0-mm, 0.5-mm, 1-mm, and 2-mm rules. With regard to the risk of nodal involvement, tumor budding has recently come to be regarded as a useful qualitative parameter together with tumor grade and vascular involvement. Furthermore, the results of a multi-institutional study (the project study by the Japanese Society for Cancer of the Colon and Rectum) has indicated that there is little risk of nodal involvement in patients with tumors whose submucosal invasion depth is less than 1,000μm.
