ダブルバルーン小腸内視鏡検査の有用性―カプセル内視鏡検査との比較

Usefulness of Double-balloon Endoscopy for Small Intestinal Diseases : Compared with Video Capsule Endoscopy

要旨　ダブルバルーン内視鏡とカプセル内視鏡の特徴を概説した．両者の検査原理の相違から，カプセル内視鏡はスクリーニングにその適応を発揮し，ダブルバルーン内視鏡は精査・治療に幅広く適応とされる．内視鏡所見と臨床診断の一致に向けての信頼性向上と，狭窄が疑われる症例への対応がカプセル内視鏡の課題であり，腸管の高度癒着症例への対応がダブルバルーン内視鏡の課題である．カプセル内視鏡診断学に，ダブルバルーン内視鏡は大きく貢献するものと思われる．また，機器や処置具の改良に伴い小腸内視鏡診療が，さらに発展することが期待される．

　The clinical features of video capsule endoscopy and double-balloon endoscopy were outlined. Depending on the examination purpose, the video capsule endoscope shows ability in screening, and double-balloon endoscopy demonstrates ability in intense scrutiny and therapeutic procedures. In video capsule endoscopy problems arise concering correspondence between endoscopic findings and clinical diagnosis, and its not being available for patients with intestinal stenosis. The safety of double-balloon endoscopy needs to be improved for application in patients with severe abdominal adhesion. We confirm that double-balloon endoscopy will make a huge contribution to the achievements of capsule endoscopy diagnostics, and further innovation will bring beneficial diagnosis and treatment for small intestinal diseases.
