要旨　免疫異常状態における消化管病変は，多彩な病像を呈するため，診断に際してはこれらの特徴をよく理解しておく必要がある．このような状態を来す代表的疾患として，AIDS，ATL，GVHDを取り上げ，代表的症例を呈示しながら特に生検診断とその際の鑑別を念頭に，各病態を概説した．病像を複雑にする要因の1つは日和見感染症であるが，病理像とともに部位・内視鏡所見が特徴的なものが多く，これらも対比しながら説明した．

　Complicated conditions of gastrointestinal lesions in cases involving immunodeficiency lead us to seek knowledge of the features of such disorders for accurate diagnosis. We select AIDS, ATL, and GVHD, which cause such conditions, and give an outline of each disease with biopsy diagnosis and differential diagnosis in mind. Opportunistic infections are one of the reasons for complicated conditions and have characteristic sites and endoscopic findings.
