要旨●TSA(traditional serrated adenoma)において,異なる組織成分をさまざまな配分で随伴するTSA(heterogeneous TSA)に注目して内視鏡的・病理学的・分子生物学的検討を行った.heterogeneous TSAは4種に分類でき,その中には,TSAに随伴する成分の一部に管状腺管と鋸歯状腺管の混在から成る分類困難な腺腫成分が存在し,KRAS/NRAS変異を特徴とする群があり,随伴成分に対し管状鋸歯状腺腫(tubuloserrated adenoma ; TubSA)の名称を提唱した.TubSAはSuSA(superficially serrated adenoma)と同様に,TSAの随伴病変・前駆病変としての意義があり,組織学的にも両者はオーバーラップしているように思われる一方で,筆者らがTubSAとした群にはSuSAの組織学的定義から逸脱するものも含まれており,概念の範囲が異なるようにも思われる.TubSAはSuSAの概念のもとでは不要なのか,SuSAの概念をより拡充するカテゴリーとして存在意義があるのか,ゲノム異常の解析も含めたさらなる検証が望まれる.
This study examines heterogeneous Traditional Serrated Adenoma(TSA), which exhibits varying proportions of different histological components. We conducted endoscopic, pathological, and molecular biological analyses to classify these heterogeneous TSAs into four subtypes. One subtype characterized by a mixture of tubular and serrated structures, along with KRAS/NRAS mutations, was designated as “tubuloserrated adenoma(TubSA)”. Similar to Superficially Serrated Adenoma(SuSA), TubSA may hold significance as either an associated or precursor lesion of TSA. While TubSA and SuSA share overlapping histological features, TubSA encompasses lesions that diverge from the histological criteria for SuSA, indicating a conceptual difference between the two. Further validation, including genomic analysis, is necessary to determine whether TubSA is redundant within the SuSA framework or represents a distinct category with its own implications.
