Abstract Purpose:We report a case of a Stage Ⅲa bleb related endophthalmitis(BRE) after trabeclectomy(TLE), in which the patient underwent vitrectomy with bleb preservation, but 4 years and 7 months later, the same wound became re-infected.

Case:77 years old, female. TLE was performed for open-angle glaucoma in the right eye. Three years later, she was diagnosed with stage Ⅲa BRE because she had a right eye corrected visual acuity of 0.3, IOP of 12 mmHg, cloudy bleb anterior chamber pus, and fibrin precipitation. Four years and seven months later, BRE(stage Ⅲa) was observed in the same wound, with visual acuity of anterior manual valve and IOP of 33 mmHg. Since it was a re-infection of the same wound site, a vitrectomy was performed with resection of the bleb. The infection was controlled, but the IOP did not decrease despite the addition of glaucoma eye drops. About a month and a half after the re-infection, a Balbert glaucoma implant insertion(BGI) was performed, and the IOP decreased to 13 mmHg at 4 months after removing the bleb.

Conclusion:Caution should be exercised in cases where the bleb is preserved at the time of initial BRE, as this may lead to recurrence.
