Abstract Purpose:To report a case of Purtscher-like retinopathy that developed following therapeutic coil embolization of an anterior communicating artery(ACA)aneurysm.

Case:A 76-years-old man who had undergone coil embolization of an anterior communicating artery aneurysm reported visual disturbances after regaining consciousness following the induction of anesthesia. The best corrected visual acuity at baseline was 0.9 and 1.2p in the right(OD)and left(OS)eyes, respectively. The IOP was 7.4 mmHg and 10.2 mmHg in the OD and OS, respectively. Relative afferent pupillary defect was not observed. Fundus examination revealed intraretinal hemorrhages, cotton wool spots, and Purtscher fleckens confined to the posterior pole in the OD. Humphrey field analyzer(HFA)revealed a visual field disturbance that corresponded to the fundus findings. Fluorescein angiography revealed obstruction of the arteriole at the posterior pole corresponding to the cotton wool spots and Purtscher fleckens. Optical coherence tomography(OCT)revealed hyperintense lesions in the inner nuclear layer of the retina. Thus, the diagnosis of Purtscher-like retinopathy was made. All lesions in the fundus had resolved almost completely after six months. OCT revealed thinning of the ganglion cell complex, which corresponded well with the visual field disturbance revealed by HFA.

Conclusion:The branching of the ophthalmic artery branches from the internal carotid artery at approximately 90° may result in endothelial dysregulation during intracranial endovascular procedures.
