はじめに

　Baggy eyelidは，加齢により下眼瞼の皮膚・眼輪筋が弛緩し，頬骨形態の加齢性変化も相まって眼窩脂肪が眼窩隔膜に包まれたまま突出することで生じる 1）2）。さらに，下眼瞼皮膚眼輪筋を眼窩骨下縁に固定するorbicularis retaining ligamentの存在により，眼窩脂肪の突出は強調され深い溝ができる 2）。この溝は内側ではtear troughと呼ばれ，外側ではpalpebromalar grooveと呼ばれる。また，下眼瞼の皮膚眼輪筋の弛緩により下眼瞼縁直下に下眼瞼溝が形成され，その周囲に複数のシワが形成される（図1）。

　Baggy eyelidに対する非手術治療として，ヒアルロン酸（hyaluronic acid：以下，HA）注入や1,540 nmノンアブレイティブフラクショナルレーザーが挙げられる 3）。また，両者の併用治療は一般的に行われている治療ではあるが，その評価を統計学的に行った研究報告はない。今回，その治療効果について，盲検化された評価者による症例写真のスコアリングにより後方視的に検証したので報告する。

　Introduction: We investigated the effects of a combined treatment with hyaluronic acid injection and 1540-nm nonablative fractional laser irradiation on changes in lower eyelid creases, tear troughs, and palpebromalar grooves (“baggy eyelids”) caused by aging.

　Methods: The combined treatment was applied in 18 participants (36 eyes). The number and depth of lower eyelid creases, and the depths of tear troughs and palpebromalar grooves were evaluated on photographs of the lower eyelids before and after treatment presented randomly to 15 evaluators. The photographs were ranked in descending order of appearance. The scores were compared before and after treatment, and were compared by the Wilcoxon signed-rank test.

　Results: The scores for the number and depth of lower eyelid creases and the depths of tear troughs and palpebromalar grooves improved significantly after treatment, as did the ranking of the patientʼs appearance.

　Discussion and Conclusion: The combined treatment with hyaluronic acid injection and 1540-nm nonablative fractional laser irradiation is effective to improve baggy eyelids and reduce lower eyelid creases, tear troughs and palpebromalar grooves.
