はじめに

　先天性眼瞼下垂症は，時として全身的合併症を背景に有することがあり，症候群の1つの症状として発症している場合もある。そのような症例における筋膜移植術の術後結果が，全身的合併症の存在しない先天性眼瞼下垂と比較して劣るのかという点については，いまだ明確なコンセンサスは得られていない。しかしながら，筋膜移植術が前頭筋の収縮に力源を求める術式である以上，筋トーヌスの低下するような疾患では術後の眼瞼挙上に何らかの影響を及ぼすことが想定される。

　本稿では当院で行っている筋膜移植術の詳細を解説し，症候群性先天性眼瞼下垂症における注意点ならびに当院での治療成績について紹介する。

　Congenital blepharoptosis can occur as a symptom of several syndromes and is sometimes accompanied by systemic comorbidities. Ascertaining the presence or absence of psychomotor retardation in congenital blepharoptosis is important, as is a general preoperative evaluation and a thorough interview of the patient regarding the presence of comorbidities in the vital organs.

　The rate of strabismus and astigmatism is higher in congenital blepharoptosis than in congenital ptosis alone. Because a delayed ophthalmological examination may severely impact the patient's visual prognosis, early consultation with an ophthalmologist is recommended.
