感染症診療の基本的考え方

感染症診療において重要な基本的考え方とは，①患者の病態が感染症であるかどうかを適切に判断すること，②感染症であると判断した場合は，血液培養をはじめとする微生物学的検査を，抗菌薬が開始される前に必ず実施すること，③迅速診断や感染部位から予想される原因微生物に有効性の高い抗菌薬を選択し，十分な効果があると考えられる用法・用量で経験的治療を開始すること，⑤原因微生物に関する微生物学的検査の結果が判明した時点で，治療効果や治療内容を再評価し，可能であれば抗菌薬のde-escalationを行うこと，⑥個々の患者背景を踏まえた治療を行うこと，である．

The basic view points of medical practice in infectious diseases are as follows : ①To judge appropriately whether a patient's condition is an infectious disease or not.

②To carry out the microbiological examinations including blood culture, before an antimicrobial therapy is initiated.

③To start empiric therapy using antimicrobial agents with appropriate dosage and administration methods that are considered to be effective for the causative microorganisms expected from the rapid diagnosis of the infectious disease or the infection site

④To reassess the treatment after the causative microorganisms become clear and to make de-escalation of antimicrobials if possible.

⑤To treat the patient based on the medical background of each individual.
