脂肪細胞と臨床検査

メタボリックシンドロームの病態の根幹として内臓脂肪蓄積が位置づけられ，脂肪細胞に関する研究が進んでいる．約10年前まではエネルギーの貯蔵庫としてのみ捉えられていた脂肪細胞が，アディポサイトカインと称される様々な生理活性物質を放出することが明らかになっている．脂肪細胞は，メタボリックシンドロームの構成要素である脂質異常症，耐糖能異常，高血圧などの動脈硬化危険因子の病態形成だけでなく，血栓や炎症を惹起し動脈硬化進展にも深く関与している．本稿では，内臓脂肪測定，アディポサイトカインの異常，脂肪細胞が深くかかわる脂質異常症，耐糖能異常，高血圧に関して臨床検査医学的見地から解説する．

Studies about adipocytes have been progressing because visceral adipose tissue has been recognized as the main pathological cause for the metabolic syndrome.　Function of adipocytes was, until about ten years ago, considered to merely store energy.　Recent studies reveal that adipocytes release various biologically active substances, which are named "adipocytokines".　Adipocytes are associated with dyslipidemia, glucose intolerance, and hypertension in the metabolic syndrome, and are also associated with thrombosis and inflammation which induce atherosclerosis.　Here, we describe a role of laboratory medicine for the understanding of measurement of visceral fat, abnormality of adipocytokines, and adipocytes-mediated dyslipidemia, glucose intolerance, and hypertension.
