I．緒言

　急性化膿性甲状腺炎は比較的稀な疾患であり，その原因や感染経路など不明な点が多い。今回われわれは63歳女性にみられた急性化膿性甲状腺炎を経験した。本患者は基礎疾患に糖尿病があり，前頸部腫脹とともに嗄声，呼吸困難が主訴であり，緊急手術のやむなきに至ったものである。幸い術後経過は順調であるが病変は気管に及び壊死を形成していた。このような気道障害を主訴とした症例はいまだ報告例をみないので，本症例の病変を呈示しその特殊性とくに糖尿病との関連について考察を加え発表する次第である。

　A rare case of acute suppurative thyroiditis with dyspnea due to tracheal necrosis is described in a 63-year-old female who has suffered from diabetes for thirty years. The patient presented with hoarseness, dyspnea and a swelling at the anterior neck. An emergency operation for tracheotomy and left lobectomy of the thyroid were performed because of dyspnea. Operative findings revealed a extended thyroid inflammation to the cricoid and the tracheal ring cartilages involving partial necrosis of these tissues. Postoperative course was uneventful with the aid of complete control of diabetes. None of this kind of extensive thyroiditis was found by our survey of literature. This is the first reported case of extensive necrotizing suppurative thyroiditis.
