I．はじめに
鼻・副鼻腔に他臓器からの転移性癌をみることは成書1）にも記載はされているが，極めて稀なものである。原発臓器の中では腎癌が多いとされているが2,3）本邦において，腎癌の鼻・副鼻腔転移症例は，筆者らが渉猟した限り，わずかに5例の報告をみるにすぎない。
われわれは重篤な鼻出血をきたし，鼻腔腫瘍の病理組織学的診断で，悪性腫瘍ではあるが判然としなかった症例が，既往歴を再検討した結果，過去に手術をうけた右腎癌（clear cell carcinorma）の病理組織像と同一であることから，腎癌の鼻腔への転移であることの判明した症例を経験した。加うるに，右腎摘後すでに2年半を経過していることから，改めて精査したところ，偶然，左腎にも腫瘤が発見されたことから，鼻腔への転移性腫瘍はこの左腎由来のものと推定された。このような臨床経過を示した鼻腔転移癌は，他に類をみず，極めて興味ある症例としてここに報告する次第である。
A case of metastatic carcinoma from the kidney in the ethmoidal region in a 74-year-old female complained of the severe nese bleeding was reported.
The patient had right nephrectomy 2 years and 6 montlls ago under the histologic diagnosis of renal cell carcinoma. Roentgenograms of the skull showed abnormal shadow of the left ethmoid sinus. CAG findings revealed an abnormal tumor shadow at the left ethmoid portion. An atempt to control the nasal hemorrhage by clipping of the left maxillary artery and left anterior and posterior ethmoid artery was successfully done. The tumor was removed by endnasal approach under local anesthesia.
The origin and metastatic route of this nasal tumor were briefly discussed.
