膠状稗粒腫—特に膠状物質の組織化学的性状について

HISTOCHEMICAL STUDY OF COLLOID SUBSTANCE IN COLLOID MILIUM

I．緒言

　膠状稗粒腫は，多数の淡黄褐色，半透明な小丘疹が集簇性に生ずるのを特色とする。1866年Wagnerが54歳女性の第1例を報告して以来，既に100例以上の報告をみるが，膠状物質の性状については一致した見解が得られていない。Ferreira-Marquesおよびvan Uden^1），Findley^2）は本物質を異常弾力線維とみなしたが，Prakken^3），Piredda^4），そしてAgius^5）は膠原線維の変性によるものとした。他方，PercivalおよびDuthie^6）は膠状物質は結合組織の変化とは無関係であつて，血液内成分に由来すると考えた。Zoon, JansenおよびHovenkamps^7），次いでBeckerおよびWilson^8）はペーパークロマトグラフィーを用いて，本膠状物質のアミノ酸組成の定性分析をおこない，膠原線維や弾力線維のアミノ酸組成よりはむしろ血清蛋白質のそれに類似することを述べた。最近，SullivanおよびEllis^9），GrahamおよびMarques^10）は組織化学的検索の結果から，膠状物質とアミロイド物質が多くの類似点をもつていることを報告した。彼等によると，膠状物質は真皮性線維芽球による異常線維性蛋白質で，コラーゲンともエラスチンとも異なる。そして，この線維性要素が酸性粘液多糖類からなる基質内に包埋されているという。われわれは最近，膠状稗粒腫の1例を経験し，膠状物質の性状について2, 3の組織化学的検索をおこなつたので，その成績を報告する。

The histochemical property of colloid substance was studied in a case of colloid milium. Alcian blue and periodic acid Schiff reaction were used for staining mucopolysaccharides and glycoprotein and/or sugarcontaining compounds respectively. Mercuric bromphenol blue reaction was utilized for detection of basic proteins. In a further attempt to identify the various polyanions histochemically, staining was carried out with alcian blue containing different concentrations of electrolytes. Methylation, saponification and diastase, hyaluronidase and sialidase digestions were also employed. The results suggest that colloid substance in colloid milium consists of a sialic acid-containing mucopolysaccharide protein complex.
