　胸郭不全症候群を伴う早期発症側弯症(EOS)に対し，拡張性胸郭形成術を併用したvertical expandable prosthetic titanium rib(VEPTR)手術がCampbellにより報告された．本手術では術後の呼吸器合併症が高頻度に発生するため，われわれは拡張性胸郭形成術を併用せず，VEPTRによる胸郭および脊柱変形の矯正術を2例のEOSに行った．その結果，術後呼吸器合併症は発生せず，脊柱変形と胸郭の矯正は良好であった．本法はEOSに対する手術法の一選択肢になりうると考えられた．

　Correction surgery with a vertical expandable prosthetic titanium rib (VEPTR) for early-onset scoliosis (EOS) originally required opening wedge thoracotomy.　However, opening wedge thoracotomy is frequently associated with postoperative pulmonary complications.　We report two cases of correction surgery with a VEPTR without opening wedge thoracotomy.　The results of the correction surgery for the spinal and thoracic deformities were favorable, and no postoperative pulmonary complications occurred in either case.　Consequently, correction surgery with a VEPTR without opening wedge thoracotomy can serve as a useful option for the treatment of EOS.
