　外傷性中大脳動脈閉塞に前大脳動脈閉塞を伴った1例を報告する。文献上，外傷性中大脳動脈閉塞症は多くの場合，初発症状として片麻痺を伴うが，本症例では失語と意識障害が主症状であった。

　外傷性の動脈閉塞の発生機序は，1）解離性動脈瘤における血管狭窄や血栓，塞栓形成が主たる原因であり，稀ながら2）脳血管れん縮によるものも報告されている。本症例では頭蓋内内頸動脈に生じた血栓が遊離して前および中大脳動脈を閉塞したものと考えられた。

We report a case of traumatic anterior and middle cerebral arteries occlusion. A 23-year-old man was admitted to our hospital following an automobile accident. He was comatose and had a small skin laceration on the left temporal region. The pupils did not react to light. CT scan revealed traumatic subarachnoid hemorrhage at the basal cistern and left sylvian fissure. His consciousness gradually recovered, but he became aphasic 4 days after the trauma. Cerebral angiography showed a irregularity of the left internal carotid artery at C1portion and occlusion of left anterior and middle cerebral artery at Al and Ml portion.
