I．はじめに

　行動の異常を主徴とする発作性自動症ictal auto—matismは，錯覚や幻覚を主徴とする発作dreamy state　seizureとともに，精神運動発作の主要な発作症状である。これらは，その焦点が多くの場合，側頭葉にあり，かつ側頭葉の病巣と密接にむすびついているため，側頭葉てんかんとも呼ばれる。

　われわれは神経学的所見，脳波上，および手術所見から，後頭葉が一次性てんかん源性焦点と考えられた症例で，臨床発作型として，大発作，発作性自動症，要素的な幻視発作を合併した症例を経験した。本例は，とくに発作性自動症が，側頭葉からはなれた脳部位の焦点によってひきおこされた点が特異的と思われ，報告する。

A case of ictal automatisms in combination with elementary visual fits and generalized seizures, 25 year-old male, was reported.

The primary epileptogenic focus in this case was situated in the right occipital lobe and seemingly free of temporal lobe. This was proved by the clinical, EEG and operative findings.

It was interesting that the epileptogenic focus of the ictal automatisms in this case was situated in the occipital region remote from the temporal lobe. These ictal automatisms were assumed to be the expression of a distant occipital spiking focus, with secondary activation of hippocampus and amygdala.
