要旨

　右心系単独の感染性心内膜炎(IE)の発生頻度は全IEの5～10％程度とされ，また右心系のIEは左心系に比して，抗生剤治療に反応が良好で，70％程度は内科的に治癒しうるとされる．患者は57歳，男性．発熱および全身かん怠感を主訴に受診した．歯科治療の既往歴があった．心エコー検査で三尖弁前尖に径13×27mmの可動性のある疣贅と中等度のTRが認められIEと診断した．PCG1,500万単位の24時間持続点滴静注を開始し，以後PCG投与を通算3カ月間行った．しかし，中等度のTRを伴った疣贅が消失しなかったため外科紹介となった．完全体外循環下に，三尖弁の弁形成術と人工弁輪を用いての弁輪形成術を施行し，術後は良好に経過した．内科的治療に抵抗性の孤立性三尖弁位のIE症例に対する１手術例を経験したので，若干の文献的考察とともに報告する．

Summary

　A 57-year-old man was admitted to our hospital for investigation of a fever of unknown origin. Blood culture and echocardiography findings revealed isolated tricuspid valve infective endocarditis. There was no evidence of any previous cardiac event but the patient had undergone a dental procedure. He was treated with antibiotics for more than 2 months,but echocardiography showed a large residual vegetation attached to the anterior cusp of the tricuspid valve with moderate tricuspid regurgitation. We performed tricuspid valvuloplasty and annuloplasty using an artificial ring. The patient had an uneventful postoperative course and continued antibiotic treatment for a further 6 weeks.
