胃潰瘍の治癒判定―X線と病理を対比して

The Healing of Gastric Ulcer

要旨　胃潰瘍・瘢痕は円形，線状，帯状の3型に分けられる．円形が基本型で，その隣接再発の反復により線状，帯状が形成される．〔治癒判定〕X線所見上，潰瘍ニッシェがなくなり，瘢痕X線像が認められた時点で，潰瘍は治癒と判定する．それで臨床的に不都合はない．なにしろ，瘢痕そのものが再び潰瘍化するという再発はないからである．〔潰瘍の治癒過程と瘢痕の経過および再生粘膜のX線像〕胃潰瘍の治癒機転には，胃壁収縮と組織補塡とがあり，とりわけ胃壁収縮の役割は大きい．潰瘍面積の98％以上が収縮により縮小する．瘢痕の経過においても収縮がみられ，瘢痕区域はなおも小さくなる．再生粘膜のX線像とその経時的変化を呈示し，組織所見と対比検討した．〔胃潰瘍取り扱い基準設定の提案〕円形潰瘍と線状，帯状潰瘍とは病態が違う．前者は単純，後者は複雑である．例えば，いわゆる胃潰瘍症とは形態的には線状，帯状潰瘍を指す．病態に差があるものは区別しなければならない．胃潰瘍の病態検討に当たり，基礎的な形態的事項の取り扱い基準が必要と考える．特に線状例の取り扱いに注目したい．そういう基準の設定を提案した．

　Gastric ulcers and scars were classified into three types: round, linear and zonal (Fig. 1). Furthermore, the round type was divided into two types : meeting type and non-meeting type (Fig. 2).

　A round ulcer healed to form a round scar. No recurrence of the ulcer from the scar itself has been observed. Recurrence of the ulcer occured in areas adjacent to or separated from the scar. Recurrences of round ulcres in neighbouring areas have been observed to form the linear type or zonal type.

　Clinically, dicision on the healing of gastric ulcer is made by the radiogical demonstration of the scar. A clear radiograph of scars can be obtained by the double contrast method.

　It should be stressed that the classification of ulcers into three types ; round, linear and zonal, is necessary in the exact diagnosis of the healing of gastric ulcers.
