要旨　膵臓は胃の背側に接して存在するため，同部の炎症や腫瘍が胃の伸展不良を惹起することがある．なかでも，膵体部から尾部にいたる広範な膵炎は，胃に波及する範囲が広く，スキルス胃癌との鑑別が問題となる．鑑別にはX線所見が重要であり，膵炎では（1）胃の背側から波及するため胃後壁大彎側の軽度の伸展不良にとどまること，（2）管外性炎症であるため体位変換で伸展不良の程度が変化すること，（3）胃壁外の石灰化を伴う場合が多いこと，などに着目すべきと思われる．臨床像から膵炎の波及を疑い，腹部超音波やCTを積極的に活用すべきことは言うまでもない．

　When inflammation and a neoplastic lesion of the pancreas infiltrate to the posterior gastric wall, we must carefully distinguish between diffusely infiltrating gastric cancer and the affected gastric lesion of pancreatitis.

　X ray examination is valuable in distinguishing extensibility of folds and location of affected lesions of the stomach. The extensibility of the affected gastric wall in pancreatitis is characterized by less severe rigidity and more changeable rigidity in the greater curvature and posterior of the gastric wall. For the clinical feature of the calcification with pancreatitis, ultrasonography and computed tomography are valuable and lead to the making of a correct diagnosis.
