要旨：地域包括ケア病棟におけるリハビリテーションの実施は包括算定であり，加えて，入院日数が60日と制限され，時限的な退院支援が求められる．当院では「しているADL」の獲得を目的に患者の傍らで個別に短時間（20分未満/回）のPoint of Care（以下，POC）リハビリテーションを提供している．今回，大腿骨頚部骨折に肺炎を併発した症例に対して，疾患別リハビリテーションとPOCリハビリテーションの介入で，「しているADL能力」を高めることができた．また，病棟内で他職種間の情報共有を行うに際し，ピクトグラムを導入して患者の能力の見える化を行い，円滑な在宅復帰支援に繋げることができた．

　Rehabilitation in the Community Comprehensive Care Unit is comprehensively calculated. In addition, the length of hospital stay is limited to 60 days, which means that patients must be discharged from the hospital on a limited basis. Our hospital provides short-term (less than 20 minutes/treatment) Point-of-Care (POC) rehabilitation individually to acquire “active ADLs.” In this study, we were able to increase the “ADL ability to do” by disease-specific rehabilitation and POC rehabilitation intervention for a patient with a femoral neck fracture and pneumonia. In addition, we were able to visualize the patient's abilities by introducing pictograms to share information among the other professions in the ward, leading to smooth support for the patient's return home.
