前立腺癌治療の功罪：アンドロゲン除去療法の問題点

Role of Androgen in the Elderly. Problems of androgen deprivation therapy.

　前立腺癌に対するアンドロゲン除去療法は有用な治療選択として認知されているが，同時にテストステロンの低下に起因した「去勢症候群（castration syndrome）」症状が出現する。性欲，性的興味の消失，勃起障害，倦怠感，知的能力の低下，抑うつ，筋力低下，腹部脂肪の増加，肉体活動，活力の低下などが，その代表的な症状とされるが，これらはLOH症候群（加齢男性性機能低下症候群）の症状とほぼ同じものである。さらに自覚症状が乏しいとしても，骨折のリスクや肥満，高血圧，糖尿病，脂質異常症からメタボリックシンドロームを呈するリスクが懸念されており，その副作用には注意を要する。

　Androgen deprivation therapy has been considered as one of useful treatment options for prostate cancer. However, it presents several symptoms of “castration syndrome” as side effects, based on low serum testosterone concentration. The symptoms includes loss of libido and sexual interest, erectile dysfunction, general fatigue, decreased intellectual ability, depression, loss of muscle strength, increased abdominal fat mass, and loss of vigor. They are almost similar to the representative symptoms of late onset hypogonadism. Furthermore, side effects such as increased risk of fracture and metabolic syndrome caused by androgen deprivation therapy should be taken care, even though subjective complaints do not appear.
