びまん性特発性骨増殖症患者に対しての冠動脈バイパス術における麻酔経験

Anesthetic Experience in Coronary Artery Bypass Grafting for a Patient with Diffuse Idiopathic Osteoporosis

要旨

　びまん性特発性骨増殖症（diffuse idiopathic skeletal hyperostosis：DISH）は，椎体前面に連続性骨化を来す原因不明の疾患である。今回，当該疾患合併患者の冠動脈バイパス術（coronary artery bypass grafting：CABG）を経験した。気管挿管のための十分な事前身体診察に加え，術中循環異常時の準備が安全な手術終了のために必要である。

　Diffuse idiopathic skeletal hyperostosis is an unknown disease causing continuous ossification in the anterior surface of the vertebral body. We experienced coronary artery bypass graft surgery for a patient with difficulty in flexion and rotation from the cervical to the thoracic spine. In addition to sufficient physical examination for intubation, preparation in case of circulatory abnormalities during chest closure is important for safe completion of surgery.
