強皮症関連間質性肺炎を伴う肺がんの術後に急性増悪を認めた1症例

A Case of Acute Exacerbation of Interstitial Lung Disease Associated with Systematic Sclerosis after Lung Cancer Surgery

要旨

　症例は74歳・女性，強皮症関連間質性肺炎を伴う原発性肺がんと診断され，胸腔鏡下左肺下葉切除術を行った。手術は順調に終了したが，術後1日目から労作時呼吸困難が出現し，術後9日目の胸部コンピュータ断層撮影（CT）で間質性肺炎像の増悪を認め，急性増悪と診断された。ステロイドパルス療法が開始され，治療が奏効し術後4カ月目に自宅退院となった。

　Interstitial lung disease often causes complications following lung cancer surgery；acute exacerbation of interstitial lung disease（AE-ILD）can be fatal. We report a case of acute exacerbation of interstitial lung disease associated with systemic sclerosis after lung cancer surgery that was improved using steroid pulse therapy.
