シミ（加齢性混在型色素斑）の治療

Treatment of Aging Complex Pigmentation (ACP)

はじめに

　本誌の読者は，形成外科医が主であると思うが，大学病院や総合病院の形成外科外来でシミの診療を行う医師はどれだけいるだろうか。著者が大学病院で診察していた35年前は，美容外科外来があったためか，少数ではあるがいた。美容皮膚科が標榜されてからは，シミは美容皮膚科や皮膚科での診療が多く，形成外科を受診する患者は少ないだろう。出向先にレーザー機器があれば，シミの治療も行わなければならない状況になるが，今では非手術の美容治療の教科書が多数出版され，またほとんどの機器にはマニュアルがあるため，治療を始める際に困ることは少ない。しかし，シミの治療は，特にレーザーなど機器を使う場合，思ったとおりの結果が出ないことや，また合併症が起こった時の説明やトラブル対処に苦労することもある。今回，30年間行ってきたシミ治療の経験や改善してきた点などを含め，現在著者が行っている方法を，症例を提示し述べる。なお，ピコ秒レーザーが出てから，Qスイッチレーザーをナノ秒レーザーと言うことも増えてきたが，今回はQスイッチレーザーと表現する。

　Nonsurgical cosmetic techniques for the face continue to progress, as they deliver good results using simple, noninvasive, and atraumatic procedures. As people age, various forms of facial skin pigmentation occur, and a mixture of melasma, seborrheic keratoses, senile lentigo, and acquired dermal melanocytosis (ADM) often occurs. We refer to this condition as aging complex pigmentation (ACP). Of course, it is important to diagnosis and treat each condition accordingly. The ACP protocol that we use consists of oral as well as topical medications. The oral medications have been tranexamic acid and vitamins C and E; the dosages are described herein. The topical medications are vitamin C, kojic acid, tranexamic acid cream, and 5% hydroquinone. UV-protection sunscreen is also prescribed, and the patients receive instructions regarding a correct facial washing technique. They are prohibited from undergoing any abrasive or mechanical massage. Once this pretreatment procedure has been followed for ≥2 months, laser or intense pulsed light (IPL) is then performed.
