 Nonsurgical cosmetic techniques for the face continue to progress, as they deliver good results using simple, noninvasive, and atraumatic procedures. As people age, various forms of facial skin pigmentation occur, and a mixture of melasma, seborrheic keratoses, senile lentigo, and acquired dermal melanocytosis (ADM) often occurs. We refer to this condition as aging complex pigmentation (ACP). Of course, it is important to diagnosis and treat each condition accordingly. The ACP protocol that we use consists of oral as well as topical medications. The oral medications have been tranexamic acid and vitamins C and E; the dosages are described herein. The topical medications are vitamin C, kojic acid, tranexamic acid cream, and 5% hydroquinone. UV-protection sunscreen is also prescribed, and the patients receive instructions regarding a correct facial washing technique. They are prohibited from undergoing any abrasive or mechanical massage. Once this pretreatment procedure has been followed for ≥2 months, laser or intense pulsed light (IPL) is then performed.
