 Some individuals in their 20s-40s were recently reported to have been dissatisfied with the results of a thread facelift and have desired a more extensive facelift. I consider Baker’s short-scar facelift with a lateral superficial musculoaponeurotic system (SMAS) ectomy to be a relatively safe and effective procedure for the lower face. However, patients in their 20s-40s have a lesser degree of sagging skin, and a short-scar facelift with lateral SMASectomy has shown a limited ability to improve the midface and nasolabial folds. At present, we perform a deep plane face lift using Aston’s finger-assisted malar elevation (FAME) technique along with the short-scar approach, and we have obtained good visual outcomes. If relatively young adults want a face lift, a technique that is effective for the midface should be adopted. Regarding facial morphology unique to Asians with wide bizygomatic distances and flatter faces, SMAS fixation suture placement at the preauricular area of the lateral face, which is suitable for Caucasians, has a weak effect on improving the midface and nasolabial folds. SMAS fixation suture placement around the line connecting the lateral canthus and the angle of the mandible has been more effective.
