オキサリプラチンによる肝類洞閉塞症候群が疑われ，その後自然軽快した1例

A case of reversible radiographic change due to sinusoidal obstruction syndrome induced by Oxaliplatin

今回我々は大腸癌に対する化学療法終了後に急性の経過で発症した門脈圧亢進症の症例を経験した。その後，原因薬剤としてオキサリプラチン（oxaliplatin：OX）が疑われ，休薬のみで自然軽快したことを確認した。OXによる副作用として肝類洞閉塞症候群（sinusoidal obstruction syndrome：SOS）が知られているが，自然に軽快した画像的変化を紹介している文献は少ない。本症例は教育的意義が高いと考えられたため，文献的考察を交えて報告する。

A man in his 50s who was diagnosed with sigmoid colon cancer（pStage Ⅲc）received 8 cycles of adjuvant chemotherapy with Capecitabine＋Oxaliplatin after Hartmann surgery and artificial anus（AA）augmentation. After the completion of chemotherapy, contrast-enhanced computed tomography revealed venous dilatation around the AA with splenomegaly, evoking a suspected diagnosis of sinusoidal obstruction syndrome. Spontaneous resolution was obtained after two months. It is important for radiologists to know the entity of reversible radiographic changes induced by Oxaliplatin-based chemotherapy.
