動脈瘤型腸管悪性リンパ腫様のCT所見を呈した小腸GISTの2例

Two cases of small intestinal GIST which showed cavitation and mimicked malignant lymphoma of aneurysmal type

消化管間質性腫瘍（gastrointestinal stromal tumor：GIST）は，小さい場合は内部が均一であるが，サイズが大きくなり悪性度が高くなるにつれて嚢胞形成，腫瘍内出血や内部壊死を起こしやすくなる^1）2）。内部壊死や腸管内腔と瘻孔形成などにより空洞形成をきたすと，一見，全周性の腸管壁肥厚をきたした動脈瘤型腸管悪性リンパ腫と類似した画像所見となることがある。今回，我々は空洞形成をきたし動脈瘤型腸管悪性リンパ腫様にみえた小腸GISTの2例を経験したので，そのCT所見の特徴について文献的考察を加えて報告する。

We reported two cases of small intestinal GIST mimicked malignant lymphoma of aneurysmal type. At first glance on CT, both tumors seemed to be circumferential thickening of the intestinal wall like malignant lymphoma of aneurysmal type. However, the tumors were confirmed to be extraluminal with exophytic growth containing internal necrotic portions and formation of a fistula into the intestine. The Absence of lymphadenopathy, inhomogeneous contrast enhancement, dynamic contrast enhancement pattern, and careful observation using MPR images may be useful for differentiating GIST from malignant lymphoma of aneurysmal type.
