開心術後早期離床を目的とした集中治療室における受動的立位の導入

Passive Orthostatism Using Tilt Table in Critical Patients after Cardiovascular Surgery

術後の早期離床やベッドサイドからの積極的な運動が退院時の日常生活動作の再獲得に効果的であることが報告されている1，2）．これまでにわれわれは，集中治療室（ICU）管理中の開心術後急性期症例に対して，心臓リハビリテーションの一環として早期離床を目的としたティルトテーブルによる受動的立位を導入してきた3，4）．その結果，受動的立位中には分時換気量の改善が得られ，酸素化低下例においては動脈血酸素分圧/吸入酸素濃度（P/F）比の有意な改善が得られることが明らかとなった．しかしながら一方で，限られた人的資源の中でティルトテーブルへの移乗やティルト中のチューブ・カテーテル類の確認，受動的立位中の呼吸・循環動態の把握を行うことに対する不安や抵抗といった，看護師や理学療法士からの意見があった．そこでわれわれは移乗を必要としないVitalGo Total Lift Bed（以下，TLB：パラマウントベッド社，東京）を導入したので，その経験を報告する．

Previously, we performed passive orthostatism using a tilt table for the purpose of early mobilization in intensive care unit patients after cardiovascular surgery. In this study, we introduced VitalGo Total Lift Bed （TLB）， which does not require patient-transfer before passive orthostatism, to reduce the burden on patients and medical staff.

No obvious adverse events were found throughout the study. In the TLB group, number of medical staff required to perform the passive orthostatism was significantly less compared to the conventional tilt table group.
