はじめに

　❶背　　景

　Ankylosing spinal disorders［ASD，強直性脊椎炎（ankylosing spondylitis）・びまん性特発性骨増殖症（diffuse idiopathic skeletal hyperostosis）］は，長いレバーアームを有することを特徴とし，軽微な外傷で不安定な脊椎骨折を発症する^1～4）．本稿では，ASDおよびASDに伴う脊椎骨折に関してレビューし，またわれわれは，胸椎および腰椎におけるASDに伴う脊椎骨折をCTとMRIを用いて骨折型を分類したため併せて報告する．

　❷ASD（AS/DISH）の定義

　ASDとは，ankylosing spondylitis（AS）とdiffuse idiopathic skeletal hyperostosis（DISH）の総称である．ASは，Modified New York criteriaで診断されるが，仙腸関節が強直しており，human leucocyte antigen（HLA）-B27が陽性である^5）．一方，DISHは，1976年にResnickにより，4椎体以上連続した椎体に骨架橋があり，椎間板狭小化，仙腸関節の強直がない疾患と報告された^6）．
