 Purpose: To clarify factors facilitating sharing information on home-care patients between home care nurses and home care workers.

 Methods: A total of 214 staff working in home care agencies, including 94 nurses and 120 workers providing home care, were surveyed using an anonymous self-reporting questionnaire on the situation of sharing information on home care patients.

 Results: As for both nurses and health care workers, more than 40% answered that the extent of sharing information among these care staff was insufficient. Both nurses and health care workers selected “feeling free to talk with my occupational counterpart” as one of factors to facilitate adequate information sharing. Moreover, as factors relevant to “feeling free to talk with my occupational counterpart”, “being affiliated with the same agency providing home care services including home care nursing” was selected by both nurses and health care workers, while “attending community care meeting(s)” was selected by nurses and “attending multi-disciplinary workshop(s)” by health care workers.

 Conclusion: In a community-based integrated care system, it was suggested that better information sharing between nurses and health care workers affiliated with different agencies can be facilitated by establishing a good relationship between these two occupations, for which interpersonal communication, provided by attending professional meetings, such as “community care meetings” and “multi-disciplinary workshops”, may be effective.
