先天性横隔膜弛緩症に対する胸腔鏡下手術の1例

Thoracoscopic diaphragmatic plication for congenital eventration of the diaphragm

　先天性横隔膜弛緩症に対する横隔膜重層固定術は安全な手技として認められている．今回，われわれは先天性横隔膜弛緩症の生後6か月の女児に対して横隔膜重層固定術を施行して良好な結果を得た．手術に際してはスワンガンツカテーテルで術側をブロックすることによって片肺換気を行い，胸腔鏡補助下に横隔膜重層固定術を行った．前腋窩線第7肋骨直上に約3cm弱の皮切を加えて第7肋骨床開胸を行った．弛緩した横隔膜をサイドクランプ鉗子で重層把持したうえ，横隔膜の重層固定を行った．術後4日目にレスピレーターから離脱することが可能となり，術後13日目に退院となった．その後，外来で経過観察を行っているが，術後経過は良好である．

Diaphragmatic plication for congenital eventration of the diaphragm is accepted as a safe procedure. We per-formed diaphragmatic plication in a six month old girl of congenital eventration of the diaphragm with a favor-able outcome. Unilateral ventilation was maintained by blocking the main bronchus by swan-ganz catheter during the surgical treatment. Then plication of the diaphragm was performed under the video asisted thoraco-scopic surgery.
