腎盂尿管腫瘍に対する後腹膜鏡下腎尿管全摘除術10例の臨床的検討

Retroperitoneoscopic Nephroureterectomy for Upper Urinary Tract Tumors:Clinical Study of 10 Cases

　腎盂尿管腫瘍に対する後腹膜鏡下腎尿管全摘除術の有用性について検討した．臨床病期TINOMOの腎盂尿管腫瘍10例を対象とした．2例は後腹膜鏡下に腎摘除を行ったのち，下腹部正中切開で直視下に下部尿管摘除を行った．8例は下腹部切開を利用したハンドアシスト法による後腹膜鏡下腎摘除と直視下での下部尿管摘除を行った．手術時間は平均449（360〜575）分，出血量は平均356（64〜1,015）gであった．合併症として腎静脈の損傷1例，肺塞栓1例，創感染2例がみられた．後腹膜鏡下腎尿管全摘除術は従来の開放手術と比べて手術時間が有意に長かったが，早期の経口摂取開始が可能であった．本術式は，腰部斜切開を必要としない点において優れた術式と思われる．

Clinical significance of retroperitoneoscopic nephroureterectomy for upper urinary tract tumors was retro-spectively evaluated. Ten cases of renal pelvic or ureter tumors with clinical stage T1N0MO were included in the present study. Two patients underwent retroperitoneoscopic nephrectomy and 8 patients underwent the same procedure but with hand-assistance through the lower abdominal incision. Resection of lower ureter with bladder cuff was performed as an open surgery via lower abdominal incision in 10 patients. The specimen was removed en bloc without opening the urinary tract via abdominal incision.
