Upside down stomachを呈した食道裂孔ヘルニアに対して単孔式腹腔鏡下手術を施行した1例

A case of single incision laparoscopic surgery for esophageal hiatal hernia showing upside down stomach

◆要旨：食道裂孔ヘルニアは比較的頻度の高い疾患であるが，胃の軸捻転を伴い縦隔内に全胃が脱出したupside down stomachを呈することは極めて稀である．今回，upside down stomachを呈した食道裂孔ヘルニアに対して単孔式腹腔鏡下手術を施行した1例について報告する．患者は76歳女性で，主訴は呼吸困難感であり，胸腹部造影CTにおいて全胃の縦隔内脱出と胃軸捻転を認めた．Upside down stomachを呈した食道裂孔ヘルニアと診断し，単孔式腹腔鏡下に食道裂孔縫縮術と胃底部横隔膜下固定およびNissen法による噴門形成術を施行した．経過は順調で，再発は認めていない．

We report a case of single incision laparoscopic surgery for esophageal hiatal hernia showing upside down stomach,which is a rare disorder caused by an esophageal hiatus hernia with gastric organoaxial volvulus.This case is a 76-year-old woman who presented with dyspnea.Computed tomography （CT） showed that the stomach had rotated and invaginated into the mediastinum.On the basis of this,we made a diagnosis of upside down stomach and esophageal hiatus hernia and performed single incision laparoscopic surgery. The esophageal hiatus was repaired,and gastropexy and Nissen fundoplication were performed.This patient was symptom free after the operation.
