手術により，神経内分泌反応と炎症反応が起こる。局所から放出された炎症性メディエータは，全身性の反応を引き起こす。炎症反応は，心房細動や，長期間に及ぶ疲労感，術後痛にも関係していると考えられている。こうした炎症反応を，非ステロイド性抗炎症薬（NSAIDs）や，リドカインなどで抑制することにより，術後痛が減弱する。
