Grasping of Environmental Impacts with Inpatients Based on Structural Modeling

はじめに

　病院に入院する患者にとって入院環境の善し悪しは，健康回復に影響する重要な要因である。看護援助にとって，患者が入院環境とどのように関わり合いながら療養生活を過ごしているかを把握することは，きわめて重要な課題である。

　これまでに患者の生活の場としての入院環境に関する研究は，患者の生活行動の実態を観察者が客観的に調査したものや，質問紙を用いた患者の意識調査，あるいはSD法による病室・病院の印象調査などにより検討されてきた^1〜9）。しかし，これらの調査では，入院環境に対する患者の相対化された共通要素の発見には至っても，個々の患者の健康回復過程に応じた具体的な看護援助計画に結びつくような研究展開にはまだ至っていないのが現状である。

　This study examined how inpatients perceive environmental impacts using the ISM (Interpretive Structural Modeling) method. 18 new items among inpatients perceiving the environmental impacts were selected through a discussion with three researchers, two nurses, and six patients. This was applied to two inpatients who were not in the same ambulatory state in order to establish the validity of the structural models.

　As a result, the structuring models showed the difference depending on each patient. Therefore the structuring model in effective to assess the environmental impacts on individuals.
