一過性脳虚血発作(TIA)は救急疾患として対処すべきであり,急性脳血管症候群(ACVS)という新たなコンセプトが提唱されている.TIAは無治療でも短時間で症状が消失してしまうため,患者や時には一般医にさえ軽視または無視されやすい.しかしながら,TIA発症早期の脳卒中リスクは従来考えられていた以上に高い.よって早期評価とリスクの層別化,およびそれらに基づく速やかな治療開始が不可欠である.最近TIA Registry.Org.という大規模な医師主導型の国際共同研究が開始され,海外諸国や本邦におけるTIA診療の実態が明らかになることが期待される.
TIA (transient ischemic attack) is a medical emergency, which has been newly termed as "ACVS(acute cerebrovascular syndrome)". TIA tends to be either unrecognized or underestimated by patients and even by their physicians since its symptoms naturally subside without any treatment. However, stroke risk is much higher than generally suspected in patients early after TIA. Therefore, rapid evaluation and risk stratification followed by immediate treatment is essential in TIA patients. Recently a large international, multicenter cooperative, observational study (TIA Registry. Org.) has been initiated, and it will yield lots of valuable clinical evidence.
