神経障害の部位と排尿障害

Neurogenic lower urinary tract dysfunction

　下部尿路は膀胱と尿道からなり，その働きは神経系に支配されている．排尿反射は脊髄―脳幹―脊髄反射で生じるが，近年の機能的脳画像を用いた研究により脳幹より上位にも高位排尿中枢が存在し，排尿反射を調節していることが明らかになった．したがって，神経の様々な部位の障害で排尿障害が生じることがわかった．一般に神経症状は障害部位により症状が異なるが，排尿症状も神経障害の部位により異なる．神経障害の部位ごとの排尿障害の特徴を知ることが，治療方針を決めるうえで重要である．

The lower urinary tract consists of the bladder and urethra, which is strictly regulated by the nervous system. Although, the micturition reflex is basically mediated by the spino-bulbo-spinal reflex, recent functional brain imaging studies have revealed that several suprapontine micturition centres regulate the micturition reflex, suggesting that various neurological disorders may cause micturition dysfunctions. Since, the pattern of micturition dysfunction depends on the location of the lesions, it is important to understand the clinical characteristics of the micturition symptoms according to the location of the lesions to determine the correct therapies.
