口内炎を初発症状とした尋常性天疱瘡の1例

A CASE OF PEMPHIGUS VULGARIS MANIFESTED IN THE ORAL MUCOSA

　I．はじめに

　尋常性天萢瘡は，全身の粘膜や皮膚の上皮内に水庖を形成し，それが破壊して易出血性びらんを生じる疾患で，皮膚科領域でもその頻度は比較的稀である。しかも本症のうち半数以上は口腔内に初発症状が生じるといわれており，その多くは耳鼻科などに受診するものと思われるが，当初は確診を得ることが困難で，"難治性潰瘍性口内炎"などとされ処置に苦慮することもあると思われる。われわれは，口内痛を主訴として耳鼻科に受診し，難治性口内炎と考えられた患者が，全身の皮膚に水庖やびらんを形成するに到り，尋常性天庖瘡と診断して副腎皮質ホルモン剤投与などにより治癒した症例を経験した。文献によると，天庖瘡が粘膜のみに限局することもあり，この場合には診断が一層困難となることが考えられる。かかる症例では，粘膜の組織検査や螢光抗体検査法を積極的に行なつて早期に診断をくだす努力の必要性があると考える。

　A 40-year-old female first complained of a pain of the palate and slight bleeding from the mucous membrane of the palate. Her dermal symptoms (bullae, erosions) began to appear about three weeks after the first oral symptoms.

　The diagnosis of pemphigus vulgaris was made by a biopsy of the skin and of the oral mucosa and by the fluorescent antibody method. She was treated with prednisolone 60 mg per day, and the dose was gradually with drown and the administration was completed in ten months. The symptoms of the skin and oral mucosa disappeared in five months, and had not recurred in the course of twelve months.
