 A normal cricopharyngeal muscle in its resting phase is provided with a weak but tonic activity. During the process of deglutition, the cricopharyngeal muscle becomes relaxed for an instant, less than 0.5 sec. when the bolus of food passes through the entrance of the oesophagus. This is immediately followed by a strong contraction. In some types of difficulty in swallowing, such as that met in cranial nerve paralysis or following partial laryngectomy, the cricopharyngeal muscle, that otherwise functions normally, may offer a significant resistance to the bolus of food that passes into the oesophagus.

 In such cases of difficulties cricopharyngeal myotomy is effective in reducing the difficulties in swallowing. Depending on the degree and extent of the causative lesions, other surgical procedures, such as a partila removal of the paretic pharyngeal wall, mediofixation of the paretic vocal cord and partila closure of the incompetant velopharyngeal region may be performed in combination along with the myotomy of the cricopharyngeal muscle.
