原発性気管円柱腫の1例

CYLINDROMA of the Trachea, A CASE REPORT

　Ⅰ．緒言

　気管原発の悪性腫瘍は非常に稀なものであり，そのなかでも円柱腫はきわめて少ない。本邦における気管原発の円柱腫の報告例は加藤ら^1）および石井ら^2）の集計によると14例の報告例があり，その後4例^{3）4）5）6）}が報告され，自験例を加えると今までに19例の報告例があるにすぎない。

　円柱腫は発育が緩慢であるため気管に特有の症状を現わすまでに比較的長期間を要するが，局所浸潤が強く，手術的に摘除しても局所再発を起こし易く，臨床上問題の多いものである。

　A man, aged 51, complained of dyspnea whenever he attempted to hold a conversation at a length of time. He soon developed irritative coughing spells accompanied by sputum which gradually took asthmatic forms. And since 8 months ago he became affected with hoarseness.

　For the relief of dyspnea, Woodman's operation was performed. Five months later the patient developed dysphagia. X-ray examination revealed an esophageal stricture about 7 cm in length at the postcricoid region which suggested malignancy.

　With the onset of further dyspnea, tracheotomy was performed which enhanced biopsy examination of the tracheal mucous membrane; cylindroma was revealed.

　The patient was treated by toal laryngectomy with added removal of the cervical esophagus. The latter was replaced by a part of the colon.
