新しいmechanical detachable coilの脳血管内手術における有用性

Usefulness of a New Mechanical Detachable Coil for Neurovascular Intervention

I．はじめに

　Electrolytically detachable platinum coil（Gu-gulielmi Detachable Coil：GDC, Target Thera-peutics社製）の開発により脳神経領域，特に脳動脈瘤に対する血管内塞栓術はより安全・確実に行えるようになった^2-4,7,8）．GDC出現以前に脳血管内手術領域で使用されてきたいわゆるフリーコイルは，一度マイクロカテーテルから血管内ないし動脈瘤内に挿入されると，簡単に回収ができないという大きな欠点があった^1,6,7）．これに対してGDCは瘤内に挿入して位置が不適当であれば回収あるいは再挿入が容易に可能であり，位置が適切であれば通電により安全に切り離すことができるという特徴をもつ^3）上このため，GDCを用いた脳神経領域の血管内治療は世界中に急速に普及している^2,7,8）．それでもなおelectrothrombosisによる不確実な血栓形成や，切り離しに時間がかかることによるembolic complicationの問題^5），さらに価格の問題等が指摘されている．

　通電によらない機械的な切り離し方式を採用したmechanical detachable coilとしてInterlockingDetachable Coil（IDC, Target Therapeutics社製）やMechanical Detachment System（MDS, Balt社製）が入手可能であるが，安全性や切り離しの信頼性に問題が残る^6，7）．日本でもGDCの普及により，これらmechanical detachable coilは使用頻度が減少してきている^7）．今回，われわれは新たにプラチナ製mechanical detachable conとして開発されたDetach Coil System（DCS, WilliamCook Europe社製）を脳神経領域の血管内塞栓術に使用したので，その経験を特にGDCとの比較検討を加えて報告する．

The electrolytically detachable platinum coil (Guglielmi Detachable Coil : GDC) is a safe and efficientendovascular tool for treatment of cerebral aneurysms. However, the GDC still has some problems, includ-ing a prolonged detaching time and high cost.

The Detach Coil System (DCS) is a newly developed platinum detachable coil for the treatment ofneurovascular diseases. This has a mechanical “screw” detachment system, which can be detached fasterthan the GDC. The platinum coil is mounted to the tip of the delivery wire by the “screw” system.
