I．はじめに

　私どもはモヤモヤ病に対して，encephalo-duro-arte—rio-synangiosis（EDAS）と呼ぶ新しい手術を開発し^11），今日までに小児モヤモヤ病50例，成人モヤモヤ病4例に対し本法を施行してきた．小児例は全例乏血性症状を示したものであり，これらに対するEDASの効果は極めて良好であり^12），本手術の支持者は年々増加している^7,15）．一方，私どもの経験した成人症例は，全例若年であるにもかかわらず3例が出血型であり，1例のみ梗塞型であった．成人モヤモヤ病，ことに出血型のモヤモヤ病に対する外科的治療法の有用性についてはいまだに確定した意見がない^14）．そこで本論文では，私どもの経験した4症例に対するEDASを中心とした間接的EC／IC bypassの手術効果を述べ，成人モヤモヤ病に対する間接的吻合術の効果について考察する．

We have performed indirect EC/IC operations, mainly the EDAS, on 50 pediatric and 4 adult moyamoya patients. In this paper, we report on the effects of the indirect EC/IC operations performed on the 4 adult patients, three females presented with intracranial hemorrhage and a male presented with infarction. The procedures performed were 6 EDAS, one EAS and a simple hematoma removal whichwas revealed later to have effected as indirect EC/IC bypass surgery. The patients were all in their twenties.
