外傷性小脳内血腫—自験例および文献的考察

Traumatic Intracerebellar Hematoma:A Case Report and the Review of the Literature

Ⅰ．緒言

　外傷性後頭蓋窩血腫は外傷性頭蓋内血腫の2-6％といわれており，その大部分を硬膜外および硬膜下血腫が占めている．外傷性小脳内血腫はさらに稀であり，全頭蓋内血腫中0.6％内外といわれており，CTの普及した今日においても報告は少ない．著者らが外傷性小脳内血腫について集め得た文献の症例数は計24例であった．またこれらの症例のうちで生存したものは13例と予後もきわめて悪かった．

　最近，私共は外傷性小脳内血腫の1例を椎骨動脈撮影およびCTスキャンにより診断し手術にて救命せしめた．この症例とともに文献的考察を加え報告する．

A case of traumatic intracerebellar hematoma in a fifty-three-year-old female is reported. She was sustained blunt trauma to occipital area being involved in traffic accident at 5:00P.M. on December 5,1977. She was admitted to another hospital where evaluation including skull radiogram was performed. Neurologically she stayed intact till 3 hours later when her speech became dysarthric and scanning. Her condition gradually deteriorated and she became semicomatose and anisocoria was noted. She was transferred to Ohmuta City Hospital at 1:00 A.M. on December 6, 1977.
