植物状態患者の社会的背景と今後の問題

I．はじめに

　Apallishes Syndrom^5）およびakinetic mutism^1）にはそれぞれの定義があることはあるが，臨床上これらがapallishes Syndromであり，これがakinetic mutismであると断定することは困難である。われわれは両者を含めてpersistent vegetative state patient^2）と呼称したほうが，臨床上より便利であると考えている。

　脳神経外科医は，しばしば意識を取り戻すことができなくなるほど脳が不可逆的変化を来たしている患者に遭遇するし，また手術によって思いもかけず術後このような状態になってしまう患者を経験するものである。われわれは現在の医学の力ではもはや意識を取り戻すことが不可能であることを知りながら，このような患者の生命を永らえるように努力するのであるが，それが長期間となると，その努力に対し無力感，無情感を感ずるのである。

It has been possible for the vegetative state patients after brain damage to survive for a long period, owing to the development of modern medical treatment. Their long survival, however, causes various social problems. For instance, they have been gradually occupying the beds for emergency, most of their families have been led to collapse, and so on.
