緊張型頭痛の発生機序

Mechanism of tension-type headache.(TTH)

　緊張型頭痛は様々な疾患が包含されているが，心因性頭痛，うつ病と緊張型頭痛とを鑑別しなければならない。これらは一見緊張型頭痛に似ているが，メカニズムもその治療法も異なるものである。

　緊張型頭痛は後頸筋の阻血性筋収縮によって起こる。そのリスクファクターにはうつむき姿勢，頭の大きさと首が細長いこと，頸椎の支持性，低血圧，貧血，ストレス，不随意の筋収縮などがあげられる。

Tension-type headache is defined as 1. Chief complaint is headache. 2. Headache is localized in the occipital area where the point of tenderness is locating. Headache in other part of the head is its referred pain. 3. Occipital muscle contraction is evident when the patient demonstrate how he begins to feel headache (during desk work, home work, or sleep) .

This type of headache is easily identifiable from the psychological headache or depression in that headache of the latter is more obscure and have with various symptoms such as sleeplessness, loss of appetite etc.
