近年開発が進む近赤外線脳機能マッピング装置（NIRS）により，非侵襲的に脳表の神経活動を観察することができるが，てんかんの焦点診断にも利用できる。発作時に焦点近傍で血液灌流が増加する現象は発作時SPECTなどで確認されているが，この現象をNIRSで捉えることができた。13例の難治性てんかんの術前診断の一環としてNIRSを行い，発作時SPECTでは明らかな増加が見られなかった4例をふくめ，全例で深部電極で確定された発作焦点に一致した部位で血液量の増加を認めた。NIRSは非侵襲的な焦点診断に有用と思われた。

Near infrared spectroscopic topography (NIRS) is a noninvasive method to measure the regional cerebral blood volume (rCBV) dynamics coupled with brain activities. We analyzed the rCBV change in the early phase of epileptic seizures in 13 patients with medically intractable epilepsy. Seizure was induced by intravenously injected bemegride in 11 cases and habitual seizures were captured in 2 cases. We used 8-channel MRS in nine cases and 24-channel in four cases. In all of the cases, rCBV increased rapidly several seconds after the seizure onset around the focus.
