小児期発症の慢性疾患の予後向上を受けて，厚生労働省は各都道府県における中核拠点となるべき移行期医療支援センターの設置を求めており，2019年以降7都府県において同センターが設置された。本論では医療体制・ネットワーク構築と自立（自律）支援の観点から千葉県移行期医療支援センターの現状，取り組み，将来像について概説する。移行医療の対象となる患者は多くの小児発症慢性疾患で増加し続けており，個人レベルでの対応は難しいため，各地域の移行期医療支援センターを核として多職種連携による医療連携・ネットワーク構築，行政への働きかけが求められている。

Abstract

The establishment of a “Transition medical care support center” in each prefecture was requested by the Japanese Ministry of Health, Labour, and Welfare in 2017. To date, seven centers have been set up, including the Chiba Prefecture Transition Medical Care Support Center at the Chiba University Hospital. This review article introduces the current status and perspectives for the development of the care support system and autonomy/independence in patients with pediatric-onset chronic disease in their transitional age. The specific network acting on the total care and support should be warranted to achieve sufficient and proper assistance to the corresponding patients.
